      Use this application (use multiple forms if you have multiple people) to submit an order for personalized book(s) or personalized aromatherapy blends, photographs or stationery.  Fill in all of the appropriate sections--be as detailed as you can.  This can be very personal information, so know that all information will be kept confidential.  Nobody but me sees this form!
     ...And don’t forget to fill out the Order Form and send the application and order form in together, along with your payment.

SECTION TWO--Storybook (8-10 weeks)                        $35/book

     The following information is needed to create a unique, personal story for your child.  It will include not only your child’s name, but also a friends’ name and some special things your child likes, plus the type of story line he or she likes ( adventure, religious, etc), so please be as detailed as you can so that Debra can create a really fun story.

     If you provide any photographs of your child or their favorite things (or best friend), Debra will incorporate them into the alphabet book.  This is optional, but photographs must be submitted  with a  photograph authorization/waiver to be considered.  Please make sure you identify your child on back of photograph. (contact us for waiver.)

     Whew!  You are done.  With all of the above information, Debra will weave a fun short story or alphabet book appropriate to your child’s age and include a few illustrations (including the photos, if provided).  Then she will bind the book, and as mentioned before, she will mail it to you within 8-10 weeks.  If there are any unforeseen delays, Debra will contact you by phone (or e-mail, if preferred).

     If you want more than one book, please make sure you complete one of these applications for each child--and please know that the turn-around time will be longer.

SECTION THREE--Photography (4-6 weeks)

pricing has yet to be established...please be patient

    To make this interesting, if you wish, we can use your photograph for note cards or post cards, but cannot guarantee quality of photographic results.  We will need a form signed authorizing use of your photograph (see Other Forms).   We will not complete order without the signed authorization OR if  we are not completely satisfied with results because of quality of your submitted photo (we will do my best) OR especially if we feel photograph is obscene or suggestive in any way.    All of your personal photographs will be returned with order whether completed or not.
